The Department of Critical Care at North Middlesex University Hospital in London offers clinical fellowships in adult Critical Care Medicine. Candidates must be fluent in spoken and written English and eligible for full GMC (UK) registration and may have specialty certification in their country of origin. The fellowship involves provision of critical care for a total of 10 level III beds and level II care for further 11 beds. There are no other areas in the hospital which admit patients with organ failure.

The case mix

The department admits approximately 1200 patients annually from medical and surgical backgrounds. Approximately 80% of the patients have underlying medical pathologies of great variety and high morbidity scores. Some of our common medical diagnoses include post-cardiac arrest, septic shock, acute coronary events and exacerbation of asthma/COPD. The Critical Care Unit offers multi-organ support including continuous renal replacement therapy, high frequency oscillatory ventilation in selected patients, full invasive monitoring and bedside echocardiography.

Outreach services

Outreach nurses trained in Critical Care, independently review patients referred to them from the wards and closely liaise with the Critical Care doctors regarding their management and plan and work as a part of the Critical Care team.

Job description of Clinical Fellow

Educational activities are the prominent feature of the fellowship. These include weekly classroom teaching, hospital grand rounds, journal clubs, case discussions and bedside teaching. Protected time for structured academic work (e.g. preparation towards EDIC or training in transthoracic echocardiography) and clinical audit activity is feasible and encouraged. The on call schedule for clinical critical care follows EU Time Directives and you will be a part of rota containing 6 other fellows. Four weeks of paid annual leave is provided per annum if applied at least 6 weeks in advance. Critical Care consultants carry out ward rounds on all admitted patients at least twice a day. The fellow is an integral part of the team, and assumes a position of considerable responsibility. Fellows are involved in all levels of patient care, including primary assessment and management, procedural work, coordination of admissions and discharges, liaison with referring services, and out-of-unit consultations. Through graded assignment of responsibility during the year, the fellow will optimally assume the role of a ‘senior registrar’. It is important that the Clinical Fellow commits to at least 6 months of working on the unit.

Situated on the North Circular Road, the hospital is within very easy reach of the main roads connecting London to the rest of UK. The hospital is situated close to the North London residential districts of Enfieldand Winchmore Hill, which have an excellent environment and schools.Silver Street main line railway station is a 5-minute walk away.
